The Local Authority will review your information or that of your loved one and take the following steps with you:
Assessment and planning
Social services typically conduct assessments to determine an individual’s care needs. Based on this assessment, they create a care plan outline your required services.
Coordination
Social services will then contact Network Care and Support to deliver the services specified in the Local Authority care plan. This can include home care, personal care and other support services.
Partnerships
We work in collaboration with the Local Authority, healthcare professionals and safeguarding teams. This ensures that care is integrated and meets your needs.
Monitoring and review
Social services will regularly review care plans and the performance of Network Care and Support to ensure that the care we provide to you or your loved one, is of the highest quality and meets your needs.
Safeguarding
Both Network Care and Support and social services work together to identify and address any risks or concerns to safeguard vulnerable individuals from harm.
