The highest quality of care and support. Here are some key aspects to consider:
Assess needs and preferences
Identify the specific care needs (medical, personal, emotional) and determine the level of care required (part-time, full-time, specialized).
Research and select provider
Look for providers registered with the Care Quality Commission CQC with good ratings
Prepare the home environment
Make the home safe and accessible by removing hazards
Communicate and plan
Establish clear communication with the care provider, share all relevant medical and personal information, and involve family members in planning and supporting the transition.
Mrs. Johnson, once active in her community, became isolated after her husband's passing. Her daughter, Sarah, sought help from Network Care and Support, who introduced Mrs. Johnson to Emma, an experienced carer.
Emma started with short walks, gradually building Mrs. Johnson's confidence. They eventually visited the community garden, where Mrs. Johnson reconnected with her love for gardening and met new friends.
With Emma's support, Mrs. Johnson began attending community events and volunteering. Her confidence grew, and she rediscovered her joy and independence, proving that with the right support, it's never too late to re-engage with life.

Thank you so much for all the wonderful care you gave to Kate over the last few months. It made all the difference to us all as a family to see her so well taken care of in such a lovely way.
I have been RN’s allocated social worker since 28th April 2023 and have been working alongside Lorraine and the carers from Topcare since. Lorraine and the staff have been excellent in meeting the needs of RN and it is evident that RN has made a great rapport with those who are directly involved in his care. The continuation of regular carers helps RN to build these relationships and because of this, Topcare have supported him with both his physical and mental wellbeing. Evidently, if RN had an agency involved in his care that was not providing the regular care that he needs, or without the continuity of familiar faces, I feel that he would not have made the progression he has done. RN has disclosed concerns in the past with his current mental state, which was safeguarded appropriately and protected within confidentiality guidelines. As such, Lorraine has supported RN with seeking medical guidance and these concerns have been minimised. I would highly recommend this agency to other individuals in the future.
